PERSONALIZED, ONE-ON-ONE CARE
Your treatment program will be based on your diagnosis, medical history, functional limitations, response to treatment, and personal goals. You will receive care from the same physical therapist throughout your rehabilitation whenever possible, promoting continuity and consistent clinical decision-making.
Physical therapy may be challenging, and some exercises may produce temporary muscular fatigue or mild discomfort. However, you should immediately inform your physical therapist of sharp pain, unusual symptoms, or any significant increase in discomfort. Your program can then be modified to maintain safety and support appropriate progress.
SUCCESS REQUIRES COLLABORATION
Successful rehabilitation requires ongoing collaboration between the patient and physical therapist. Patients are encouraged to:
Provide a complete and accurate medical history, including current medications, previous illnesses, surgeries, hospitalizations, and other information relevant to their condition.
Participate in the development of the treatment plan and ask questions whenever instructions or recommendations are unclear.
Attend scheduled appointments and provide reasonable notice when cancellation or rescheduling is necessary.
Follow the agreed-upon home exercise program and report any adverse response.
Inform the physical therapist of changes in symptoms, health status, medications, or functional abilities.
Meet applicable financial responsibilities, including copayments, deductibles, coinsurance, or self-pay charges.
Follow clinic policies and infection-control precautions intended to protect patients, visitors, and staff.
