Thank you for choosing Campus Physical Therapy, Inc. We understand that you have many options for physical therapy, and we appreciate the confidence you have placed in us. The following information will help you prepare for your appointments and understand our office policies.
DOES MY INSURANCE COVER MY SERVICES?
Many health insurance plans provide coverage for medically necessary physical therapy. However, benefits vary according to the specific plan and may include referral, authorization, visit-limit, copayment, coinsurance, or deductible requirements.
Verification of benefits does not guarantee payment by your insurance plan. Patients remain responsible for applicable copayments, deductibles, coinsurance, noncovered services, and other amounts assigned by their insurance plan. Please contact your insurance company or our office if you have questions about your coverage.
WHY IS REGULAR ATTENDANCE IMPORTANT?
Consistent attendance allows your physical therapist to monitor your condition, evaluate your response to treatment, and progress your rehabilitation program safely. Frequent cancellations, missed visits, or irregular attendance may delay recovery and may result in suspension or discontinuation of care.
Please arrive on time and provide at least 24 hours’ notice if you need to cancel or reschedule an appointment. A late-cancellation or missed-appointment fee may apply according to clinic policy. Missed appointments also prevent other patients from using the reserved treatment time.
WHAT SHOULD I WEAR?
Dress properly. Remember that you are coming to receive treatment and most likely you will need to expose the treatment area. Please use loose clothing such as short pants, sleeveless T-shirts, tank tops. Bring your glasses or earing aids if you need them.
SHOULD I PERFORM MY EXERCISES AT HOME?
Yes. Active participation is an important part of physical therapy. Your physical therapist may prescribe exercises and activities based on your condition, abilities, and treatment goals.
Perform the exercises only as instructed. Mild muscular effort or stretching may be expected, but you should not continue an exercise that causes sharp pain, significant symptom aggravation, dizziness, or another unusual response. Inform your physical therapist about any problems so the program can be modified appropriately.
WHAT SHOULD I BRING TO MY FIRST VISIT?
Please arrive 15–20 minutes before your scheduled appointment if you need to complete registration paperwork. When available, forms may be completed in advance.
Please bring:
- A government-issued photo identification card
- Your current insurance card
- Your physician’s referral, prescription, or medical diagnosis documentation, when applicable
- Relevant operative reports, imaging reports, or medical records
- A list of your current medications
- Any brace, orthotic, cane, walker, or other device related to your condition
- A method of payment for applicable copayments, deductibles, or other patient-responsibility amounts
WHAT DOCUMENTS SHOULD I BRING TO MY FIRST VISIT?
Please allow a 15-20 minutes in advance if your are visiting our office for the first time. There is always several papers to be completed. It is our preference that you complete your documentacion online or by downloading our intake forms. Please also bring the following documents: your identification card, your insurance card, the referral script or a medical diagnosis note. Please also bring a form of payment such as credit card, check or cash for your co-payment or deductible amount.
CAN I RECEIVE PHYSICAL THERAPY WITHOUT A PHYSICIAN’S REFERRAL?
California law permits eligible patients to begin physical therapy without first obtaining a physician’s referral. This is known as direct access.
For patients who do not already have an applicable medical diagnosis, direct-access treatment is generally limited to 12 visits or 45 calendar days, whichever occurs first. Continued treatment beyond that limit requires satisfaction of applicable California requirements, including involvement of a physician, surgeon, or podiatrist.
Insurance coverage rules are separate from California direct-access law. Your insurance plan may still require a referral, authorization, certified plan of care, or other documentation before it will pay for physical therapy. Please contact our office before scheduling so that we can review the applicable requirements.
